ConnextCare cares about our patients and the services they receive. If you are not completely satisfied with the care we provide, we want to offer you the opportunity to voice your concern.
We respect your feelings and opinions and will use them to help us serve you better in the future. Feel free to email or submit the following form:

If you are not satisfied with the center’s response, you can voice your opinion to the NYS Health Department at 1-800-804-5447 or the Joint Commission at 1-800-994-6610, or email them at complaint@jointcommission.org.
Transferring Records
It is critical that you complete the Request for Information form, located in the Forms section, if it was not completed at the time of your new patient paperwork. This allows us to request your medical records from your current or previous providers and helps us provide continuity of care.
This form should be completed for your current or previous primary care provider, as well as each specialist you may see. Please return completed forms to our office within two weeks of your scheduled appointment to allow enough time to secure your records.

Our patients’ privacy is a priority. We respect your privacy and strictly adhere to the Health Insurance Portability and Accountability Act (HIPAA).
