Preventive Care & Community Health Screenings
Preventive screenings help detect many conditions early — often before symptoms begin —
when treatment is most effective. Your ConnextCare provider can help determine which
screenings are appropriate based on your age, medical history, and individual risk factors.
Please note: ConnextCare providers can order recommended screenings
and testing. These services are completed at outside laboratories, imaging centers,
or specialty facilities.
Cancer Screening Orders
Regular cancer screenings can help detect disease at an early stage, when treatment is most
successful. Screening recommendations vary depending on age, family history, and risk factors.
Colorectal Cancer Screening
Adults age 45 and older should discuss colorectal cancer screening with their provider.
Screening can detect precancerous changes or early cancer before symptoms develop.
- Colonoscopy referrals
- Fecal occult blood testing (FOBT)
Breast Cancer Screening
Mammograms can help detect breast cancer early. Women age 40 and older,
or those at increased risk, should review screening recommendations with their provider.
Cervical Cancer Screening
Pap smears help detect cervical cancer and precancerous changes.
Screening is typically recommended beginning at age 21 and continues at regular intervals.
Prostate Cancer Screening
Men age 50 and older — or earlier for those at increased risk —
should talk with their provider about the benefits and considerations of PSA testing.
- Prostate-specific antigen (PSA) blood testing
Lung Cancer Screening
Annual low-dose CT screening may be recommended for adults ages 50–80
with a significant smoking history. Your provider can help determine eligibility.
- Low-dose CT scan referrals
STD / STI Testing Orders
Sexually transmitted infections often have no symptoms. Routine testing is an important
part of preventive health care and may be recommended based on age, risk factors, or symptoms.
- Gonorrhea and Chlamydia (GC/Chlamydia)
- HIV testing
- Hepatitis screening
- Syphilis testing
Lyme Disease Testing Orders
Lyme disease is transmitted through tick bites and is common in our region.
If you have experienced a tick bite or symptoms such as fever, fatigue,
joint pain, or rash, speak with your provider about whether testing is appropriate.
Early evaluation and treatment can help prevent more serious complications.

- Complete testing at an approved laboratory or imaging center.
