ConnextCare offers telehealth visits, allowing patients to connect with their provider conveniently and securely from home.
Using a smartphone, tablet, laptop, or computer, patients can meet with a provider for many healthcare needs without coming into the office.
To schedule a telehealth visit or request that an existing appointment be changed to a virtual visit, please call or email your health center. Be sure to include your name, date of birth, phone number, reason for visit, preferred provider, and preferred appointment time.
