Mental Health Services
ConnextCare provides integrated mental health services to support the emotional wellness, behavioral health, and overall care needs of patients.
Mental health services are integrated at each of the six main ConnextCare locations — Central Square, Fulton, Mexico, Oswego, Phoenix, and Pulaski.
Counseling Services
Licensed Clinical Social Workers (LCSWs) provide on-site services to patients of all ages through a referral from an established patient’s primary care provider. LCSWs collaborate with ConnextCare primary care providers to inform patient treatment plans and support patients through short-term or long-term therapy.
Psychiatry Services
Psychiatry services are available on site for adults and in our Pulaski office for pediatric patients. These services are fully integrated within the primary care office. Psychiatrists work with primary care providers and LCSW staff, offering consultative support for medication management as well as collaborative models of care to help improve health outcomes.
All major insurances are accepted, and a sliding fee scale discount program is available for eligible patients.
