ConnextCare School-Based Health Centers bring convenient, coordinated healthcare directly into participating schools, helping students access the care they need in a familiar setting during the school day.

Healthcare Built Into the School Day

ConnextCare's School-Based Health Center program helps students access healthcare in a familiar and convenient setting—their school.

Once enrolled, eligible students can receive healthcare services from ConnextCare professionals during the school day, helping families reduce time away from work while helping students spend less time away from the classroom.

Our School-Based Health Center teams work with students, families, school staff and other healthcare providers to support each student's physical, emotional and oral health.

Whole-Person Care for Students

Depending on the school location, ConnextCare School-Based Health Centers may provide primary medical care, mental health services and dental care. Services available may vary by school.

Primary Medical Care

Students can access age-appropriate primary and preventive healthcare, including care for illness or injury, health assessments, routine follow-up and other healthcare needs.

Mental Health

Mental health professionals provide counseling, support and coordinated behavioral healthcare in a setting that is familiar and accessible to students.

Dental Care

Dental services are available at participating schools, helping students receive important preventive oral healthcare without having to leave the school setting.

How the Program Works

Getting connected to school-based healthcare is simple.

Enroll Your Student

A parent or guardian completes enrollment before School-Based Health Center services can be provided.

Access Care at School

Once enrolled, students can access available ConnextCare services through their participating school location.

Stay Connected

Our SBHC teams communicate with families and coordinate with other healthcare providers when appropriate to support continuity of care.

Enrollment & Cost Information

Families should know what to expect before their student receives care through the School-Based Health Center program.

Enrollment Is Required

Students must be enrolled in the School-Based Health Center program before receiving SBHC services.

Enrollment allows students to access available services through their participating school throughout the school year.

No Out-of-Pocket Costs for SBHC Visits

School-Based Health Center services will be provided to students with no out-of-pocket expenses to students or their families, but third-party reimbursement will be sought.

Assistance with eligibility determination and enrollment in Child Health Plus, Medicaid or other health insurance coverage will be provided for those students with no healthcare coverage.

You may receive an Explanation of Benefits (EOB) from your insurance company, but you will not receive a bill from the SBHC.
