Care is closer when it is part of the school day. ConnextCare brings medical, mental health and dental services to participating schools, helping students get the care they need while spending less time away from the classroom.

ConnextCare is expanding school-based mobile dental services to 11 additional schools in November 2026, bringing convenient dental care to more students in the Central Square, Fulton, Mexico and Oswego school districts.

Healthcare Built into the School Day
Our School-Based Health Centers (SBHCs) connect students with ConnextCare healthcare professionals right at school.
From a visit for an illness to counseling or preventive dental care, school-based services help families manage their child's health with fewer trips and less time away from work. Available services vary by school.
Our teams work with students, families, school staff and other healthcare providers to support each child's physical, emotional and oral health.
Whole-Person Care for Students
Depending on the school location, ConnextCare School-Based
Health Centers may provide primary medical care, mental health
services and dental care. Services available may vary by school.
Primary Medical Care
Students can access age-appropriate primary and preventive
healthcare, including care for illness or injury, health
assessments, routine follow-up and other healthcare needs.
Mental Health
Mental health professionals provide counseling, support
and coordinated behavioral healthcare in a setting that is
familiar and accessible to students.
Dental Care
School-based dental services help students receive preventive oral healthcare in a familiar setting. Our mobile dental program will reach 11 additional schools beginning in November 2026.
How the Program Works
Getting connected to school-based healthcare is simple.

Enroll Your Student
A parent or guardian completes the appropriate SBHC enrollment form before services begin. Enrollment for the new mobile dental sites is available now.

Access Care at School
Once enrolled, students can access available
ConnextCare services through their participating
school location.

Stay Connected
Our SBHC teams communicate with families and coordinate
with other healthcare providers when appropriate to
support continuity of care.

receives care through the School-Based Health Center program.

Parents and guardians can enroll now for current school-based services and for mobile dental services coming to the 11 new school locations in November 2026. Select the form for the services your child needs.

No Out-of-Pocket Costs for SBHC Visits
School-Based Health Center services will be provided to
students with no out-of-pocket expenses to students
or their families, but third-party reimbursement
will be sought.
Assistance with eligibility determination and enrollment in
Child Health Plus, Medicaid or other health insurance
coverage will be provided for those students with
no healthcare coverage.
You may receive an
Explanation of Benefits (EOB) from your
insurance company, but you will
not receive a bill from the SBHC.
Mobile Dental: Coming November 2026
These 11 new locations will offer school-based mobile dental services. Parents and guardians can enroll their children now so enrollment is in place before services begin.
