What Does It Cost?

There are no out-of-pocket costs for SBHC visits.

The SBHC bills most insurance plans and accepts the insurance payment as full payment. Families are not responsible for co-pays, deductibles, or visit balances.

You may receive an Explanation of Benefits (EOB) from your insurance company, but you will not receive a bill from the SBHC.

What Costs Am I Responsible For?

While visits to the SBHC are free of charge, families may be responsible for costs related to:

• Prescription medications
• Laboratory tests
• X-rays
• Specialty referrals or appointments

How Do I Enroll My Child?

All school-aged children are eligible to participate.

To enroll, you must complete an enrollment form for each child. Forms are available:

• On this website
• At your child’s school nurse’s office
• At the SBHC located in your school

What If My Child Is Sick?

If School Is Open

You may call the SBHC to schedule an appointment or walk in for care.

If your child becomes sick or injured during the school day, they can visit the school nurse. If additional care is needed, the nurse will contact you and help coordinate an appointment with the SBHC.

If School Is Closed

SBHCs operate only when school is open.

When school is closed, students may receive care at any ConnextCare primary care location. Standard charges may apply for visits at these locations.
