What is a School-Based Health Center (SBHC)?
A School-Based Health Center (SBHC) is a health clinic located within a school that provides
convenient, high-quality care for enrolled students during the school day.
SBHCs are operated by ConnextCare and are partially funded by the
New York State Department of Health.
Each SBHC is staffed by a Nurse Practitioner or Physician Assistant, with a
physician available for consultation when needed. Services are designed to make it easier for
students to receive care without missing significant classroom time.
All ConnextCare SBHCs offer integrated Primary Care, Mental Health, and Dental services
to support the overall health and well-being of students.
Primary Care Services
Primary care services are available to students enrolled in the SBHC program and include:
- Comprehensive physical examinations following American Academy of Pediatrics guidelines
- Monitoring and management of chronic conditions such as asthma and allergies
- Administration of immunizations
- Treatment of minor injuries
- Diagnosis and treatment of common illnesses
- Prescription medications sent to the pharmacy of your choice when needed
Dental Services
Preventive dental services are available at participating SBHC locations and may include:
- Dental cleanings
- Fluoride treatments
- Dental sealants
- Oral health education
Mental Health Services
Mental health support is available for students and their families and may include:
- Individual counseling
- Psychoeducational and behavioral case reviews
- Family counseling
What Does It Cost?
There are no out-of-pocket costs for SBHC visits.
The SBHC bills most insurance plans and accepts the insurance payment as full payment.
Families are not responsible for co-pays, deductibles, or visit balances.
You may receive an Explanation of Benefits (EOB) from your insurance company, but you will
not receive a bill from the SBHC.
What Costs Am I Responsible For?
While visits to the SBHC are free of charge, families may be responsible for costs related to:
- Prescription medications
- Laboratory tests
- X-rays
- Specialty referrals or appointments
How Do I Enroll My Child?
All school-aged children are eligible to participate.
To enroll, you must complete an enrollment form for each child. Forms are available:
- On this website
- At your child’s school nurse’s office
- At the SBHC located in your school
If you need help completing the enrollment form, our team will be happy to assist you.
What If My Child Is Sick?
If School Is Open
You may call the SBHC to schedule an appointment or walk in for care.
If your child becomes sick or injured during the school day, they can visit the school nurse.
If additional care is needed, the nurse will contact you and help coordinate an appointment with the SBHC.
If School Is Closed
SBHCs operate only when school is open.
When school is closed, students may receive care at any ConnextCare primary care location.
Standard charges may apply for visits at these locations.
