Sliding Fee Discount Program
ConnextCare provides care to all patients, regardless of their ability to pay. Our Sliding Fee Discount Program adjusts the cost of medical and dental services based on your household income and family size.
If you have insurance, the program applies only to the portion you are responsible for paying, excluding insurance co-payments.
Additional Benefits
- Helps cover a portion of lab and pharmacy costs for uninsured patients
- Pharmacy benefit available for qualifying Medicare patients without secondary insurance
- Lab services must be performed through Oswego Health laboratories
- Prescriptions and lab work must be ordered by a ConnextCare provider

You may also request an application during your next visit. Verification of all income sources is required and may include your most recent tax return, pay stubs, unemployment documentation, or Social Security/Public Assistance statements.
